Trouble with fan controller.

I have two fan controllers that control 4 fans each... they worked great, and well still do. But I found them operating oddly a few times. They would not control one of the fans. Like the fan would not turn off and it just turn at max RPM... Is this a porblem with the controller? fan? or the PSU?
Note* I use an alternate PSU then the one running my computer.

Is this just one fan out of 4 on the same controller that's acting up? If so I'd suspect either the fan or the Pot from the controller.

I would go with the pot... see I can hook any fan to it and it will being to spin out. the odder thing is. I have 2 of these fan controllers, and both of them have the same problem.
